Below is a list of the forms for the High School.

 

Directory Information - This form must be filled out annually for all students. *Required

Free/Reduced Meal Application - This form must be filled out annually no later than October 1st.

Health Information/Emergency Plan - The Health Information form must be filled out annually.  The Emergency Plan form is only necessary if there are specific needs listed on the Health Information form. *Required

New Student Data - This form is needed only for students who are new to the Kingsland School District.

Over-the-Counter Medication - This form must be filled out in order for students to take over-the-counter medication (Tylenol, Advil, cough drops, etc.) at school.  Parent/Guardian signature is required.

Prescription Medication - This form must be filled out in order for students to take prescription medication at school.  The form must be signed by the parent/guardian and the prescribing physician.
